Here is a basic template for the information that should be included in an invoice. When to invoice, how much to invoice for and payment terms should be agreed through a contract or other written agreement.

INVOICE REFERENCE NAME/NUMBER
(one that makes sense to you to help you keep track of invoices, it could be as simple as no.001)

YOUR NAME
YOUR ADDRESS

YOUR UTR NUMBER
(Unique Taxpayer Reference - you get one of these by registering as self-employed with HMRC, which you can do online)

NAME OF THE PERSON/ORGANISATION THAT YOU ARE INVOICING
ORGANISATION ADDRESS

DATE

DESCRIPTION OF SERVICES PERFORMED

FEE DUE
(This might be broken down into different fees and expenses, depending on what has been agreed, but should always include a total)

YOUR BANK DETAILS

ATTACH COPIES OF ANY EXPENSES TO YOUR INVOICE (where these have been agreed beforehand)
